Reporting of Suspected Adverse Reactions

You can report any suspected adverse reactions via the national reporting system through the Adverse Event Following Immunisation (AEFI) Reporting Form available from the National Adverse Drug Reaction Monitoring Centre (NADRMC) and the nearest government pharmacy facility (hospital/health centre) and include batch/Lot number if available. The completed AEFI Reporting Form should be returned to the nearest government pharmacy facility (hospital/health centre) or e-mail at nadrmc.dps@moh.gov.bn.
